The 5-Day Miracle: Understanding Azithromycin (Zithromax) Dosing

Azithromycin, widely known by the brand name Zithromax or simply "Z-Pack," is one of the most popular antibiotics in pediatric medicine. Its popularity stems from its incredible half-life—allowing for a short 5-day course to treat infections that would require 10 days of other antibiotics.
However, calculating the dose is slightly more complex than a standard antibiotic because the "loading dose" (Day 1) differs from the "maintenance dose" (Days 2-5).

The Standard Protocol: 10/5/5/5/5

For most common pediatric infections, such as community-acquired pneumonia, pharyngitis (strep throat), or acute otitis media (ear infections), dosing is strictly weight-based.

Day 1 (Loading Dose): 10 mg/kg
This higher initial dose is designed to "saturate" the body's tissues quickly, reaching therapeutic levels within 24 hours. Because Azithromycin concentrates in white blood cells, it is actively transported to the site of infection.

Days 2 through 5 (Maintenance Dose): 5 mg/kg
These subsequent doses maintain the level of the drug in the system. Because the body eliminates Azithromycin very slowly (it can stay in the system for up to 10 days after the last dose), shorter daily dosing works effectively.

One-Day Dosing (Massive Dose)

In some specific cases, such as certain ear infections, a doctor might prescribe a single, massive one-time dose of 30 mg/kg.
- Pros: Greatest compliance (you only have to fight with the kid to take medicine once).
- Cons: Highest risk of stomach upset (vomiting, diarrhea).
This calculator focuses on the standard 5-day regimen, which is gentler on the stomach.

Strep Throat Exception

While the 5-day course is common, treatment for Group A Streptococcus (Strep Throat) typically requires a total cumulative dose of 60mg/kg over the course of treatment, or a specific 12mg/kg for 5 days protocol. Always confirm the diagnosis. Azithromycin is rarely the first choice for Strep (Amoxicillin is preferred) due to rising resistance rates.

Liquid Concentrations

Azithromycin suspension usually comes in two strengths:
- 100 mg / 5 mL: Often used for smaller babies.
- 200 mg / 5 mL: Standard for toddlers and children.
Double-check the bottle. If the calculator says "Give 200mg" and you have the 100mg/5mL bottle, you need to give 10 mL. If you have the 200mg/5mL bottle, you only give 5 mL.

Why No Refrigeration?

Unlike Amoxicillin or Augmentin, liquid Azithromycin should NOT be refrigerated. It essentially turns into a thick, bitter sludge if it gets cold. Store it at room temperature and throw away any unused portion after 10 days.
